1. Couple Profile

Name of the person filling out this form.
Names of the bride and groom / couple.
Main contact email.
Phone number with country code.
Date of the wedding.
Name of the hotel, villa, beach club or private venue.

2. Event Logistics

Name of the planner or person in charge on the wedding day.
Direct phone number for event-day communication.
Google Maps or exact venue location link.
Complete physical address of the event.
What time does the wedding or event officially start?
What time is the event expected to finish?
What time can the DJ team access the venue to start setting up?

3. Technical Requirements

Select the type of space where the DJ setup will be placed.
Is there parking close to the DJ setup area?
Approximate distance from parking/loading area to the DJ setup.
Mention stairs, sand, elevators, long walks, uneven paths, etc.
Is stable power available close to the DJ setup?
DJ Setup Notice: The DJ requires 2–3 hours before the event.
Parking Notice: Parking close to the setup area is required.
Electricity Notice: Stable power must be available.

4. Musical Vision

Select up to 3 options that best describe your desired atmosphere.
Describe your guests: nationalities, vibe, music taste, family/friends dynamic.
Approximate age range of most guests.

5. Ceremony Experience Design

Name of the person leading the ceremony.
Playlist link or vibe while guests arrive before the ceremony.
Song for parents, family or special relatives.
Song for bridesmaids, groomsmen or wedding party.
Main entrance song for the bride or couple.
Song or cue immediately after the kiss.
Recessional song as the couple exits.
How many microphones are needed? For officiant, vows, readings or musicians?

6. Key Wedding Moments

Song name and version: full, short or custom edit.
Song for father-daughter dance, if applicable.
Song for mother-son dance, if applicable.
Song for cake cutting moment.

7. Cocktail & Dinner

Choose the atmosphere for cocktail hour.
Choose the energy during dinner.
Spotify, Apple Music, YouTube or any reference playlist.

8. Party Energy Design

Song to officially open the dance floor.
Select the styles that should guide the party.
How intense should the highest point of the party feel?
Specific songs that must be played during the event.
Songs, artists or genres that should be avoided.
Reference playlist for the dancing part of the wedding.
